Penis size is a genetic trait like any other.

However, the penis may seem disproportionately small on an obese male. It may also be shrouded in adipose (Eg. "fat") tissue.

Also, if there is peripheral vascular problems (obesity -> hypertension, obesity -> diabetes), both requiring medication, and as a result a full erection may be problematic.
