NEW PATIENTS |
Dr. Adams' patients are
referred to this office from their primary physician or other established
referral source. On a very limited basis, we will accept self-referred patients.
When an appointment is scheduled, all new patients receive an introductory
letter as well as a map with directions to our office. Please be sure we have
your correct address and telephone contact information on file at the time of
scheduling.
The purpose of the initial visit is to determine the course of care most likely
to benefit you. It is important to remember that this initial visit will be used
to obtain a complete family, developmental, medical and occupational history.
Upon arrival, you will fill out a history and consent form. Please read it
carefully to understand your privacy rights under HIPAA guidelines.
Your first visit will consist of a clinical interview with Dr. Adams, at which
time you will have the opportunity to share any information you feel is
relevant, as well as answer the questions which will help Dr. Adams understand
your situation.
The Medical Quarters is a non-smoking environment.
